Who can refer someone to Ready Rooms?

Anyone coordinating housing on behalf of a client can submit a professional referral: hospital discharge planners, social workers, case managers, behavioral health and substance use treatment providers, VA and veteran service organizations, FQHCs, home health and hospice teams, skilled nursing facilities, reentry and probation programs, domestic violence organizations, and government or nonprofit agencies.

Why should hospitals and agencies partner with Ready Rooms?

Unsafe discharge and housing instability drive readmissions and poor outcomes. Ready Rooms gives hospitals, behavioral health providers, and agencies a single housing navigation partner across Texas, reducing the time discharge planners and case managers spend calling around, and giving clients a documented path from referral to housing options.

What happens after I submit a referral?

A coordinator reviews it and follows up with you by phone, text or email about status, options, or missing information. Coordinators recommend matches and send the client's information to a provider only under the consent you attest to on the form. Providers are independently operated and make their own acceptance decisions, so Ready Rooms does not guarantee housing or placement.
