- I am requesting housing-navigation and placement assistance from Ready Rooms.
- Ready Rooms may review my submitted information and identify potential housing providers.
- Ready Rooms may share my approved referral information with multiple housing providers for housing consideration.
- Those providers may contact me directly by phone, email, or text regarding availability, screening, tours, and move-in.
- Submitting a request does not guarantee housing, availability, acceptance, or placement. Housing providers make their own final acceptance decisions.
- Ready Rooms may contact me to verify referral outcomes, tours, and move-in.
- Ready Rooms may retain my submission, this consent record, my referral history, and placement outcome for operational and compliance purposes.
- Sensitive information about me is only shared with a provider when I specifically authorize it or when required under Ready Rooms' approved disclosure policy.
For individuals requesting home care information
By submitting a home care request to Ready Rooms, I confirm that:
- I am requesting information about home care options through Ready Rooms, and I understand Ready Rooms does not provide home care directly.
- My submitted information (name, phone, email if provided, city, state, and the care details I share) is reviewed by a Ready Rooms team member, who may follow up with me and, where appropriate, share my information with a home care provider or resource in Ready Rooms' network so they can contact me.
- Ready Rooms may contact me by phone, email, or text to follow up on this request.
- Submitting a request does not guarantee that home care will be found, is available, or is affordable, and does not guarantee any provider's acceptance.
- Ready Rooms may retain my submission and this consent record for follow-up and operational purposes.
For professionals submitting a referral
By submitting a referral to Ready Rooms, I confirm that:
- I have informed the client that Ready Rooms may share this referral with multiple housing providers.
- I have the client's permission, or documented authority, to submit and disclose this information.
- Providers may contact the client or the identified case manager directly, depending on the contact method I've selected.
- The information I am submitting is accurate to the best of my knowledge.
